Help us understand the person’s goals, location and the support they are seeking. With their consent or appropriate authority, you can make an initial referral without uploading their full NDIS plan or clinical records. Nominate who we should contact first. We’ll review the referral and discuss whether our services and current availability are a suitable match.
A clear starting point.
Who can refer?
Family members, carers, guardians, friends, advocates, support coordinators, health professionals and other service providers can make a referral with the participant’s consent or appropriate authority. If you are seeking support for yourself, make a general enquiry.
What information is useful?
Provide your name, relationship or professional role, preferred contact method and corresponding email address or phone number. Add the participant’s suburb or postcode and one or more requested supports, or choose “Not sure”.
The optional message can briefly describe goals, preferred times or communication needs. An optional organisation field appears for professional roles. You do not need to upload documents; this form has no upload field.
Consent comes first
Before submitting, confirm that the participant agrees to the referral and contact with the person selected, or that you have appropriate authority to act on their behalf. Share only what is needed for this initial conversation.
Who will we contact first?
Choose yourself as the referrer, the participant or another nominated person. If you choose someone else, the form asks for their name, preferred contact method and email address or phone number. RHC will use that selection for the initial follow-up.
How do we check the fit?
Our team reviews the requested support and location, then discusses individual needs, preferences, appropriate staffing and current availability. Sending a referral does not book support, confirm funding or guarantee a service.
If we need more information
We’ll discuss what is needed, why it is relevant and how it can be provided securely, with the participant’s consent or appropriate authority. Please leave out NDIS numbers, full plans, diagnoses, clinical records and identity documents from the initial form.
