Check notice periods, cancellation terms and any outstanding payments. Ask for clarification before agreeing on an end date.
Discuss availability with the new provider and plan dates to reduce gaps. Tell your coordinator or plan manager, if you have one. Confirm any provider relationships or bookings that need updating in your NDIS portal.
Ask for a written service agreement that sets out what you have agreed with your new provider.
